Shopko Pharmacy Closing June 6

Effective June 6, at 4 p.m, Shopko Pharmacy will be closing. The Shopko main store will continue to remain open and operating as usual.

Starting June 7, 5th Avenue Pharmacy & Gift will be able to fill prescriptions and refills for Shopko customers. Records and files will be transferred, as well as current insurance information.

It is recommended, however, to carry your insurance information when receiving prescriptions from 5th Avenue Pharmacy & Gift for the first time. Patients should also phone in for their refills a few days in advance of needing their medication to give the pharmacy time to fill their order timely and to make sure the medication is in stock, especially if it is a high dollar specialty item. You may also use the phone app, PocketRx, to request your refill.

5th Avenue Pharmacy & Gift is not a Medicare Part B supplier and does not bill Medicare for diabetic testing supplies or inhalation solutions. Patients can call to discuss their options. The pharmacy is contracted with most Medicare Part D and other major pharmacy prescription benefit plans, but patients should check with their plans to check on preferred status.

5th Avenue Pharmacy & Gift does offer mail out and local delivery services. The cost for mailing most prescriptions is a flat $4.00 charge. Additional charges may apply to large requests or special requests.
